Hi @JEKA thx for feedback. Well I cant tell you an exact protocol how i improved my erection and sperm quality back. Even my sexual desire has risen a bit, along beard growth etc. So I think my DHT as well kicked back funtioning on tissues. Imo there are 2 types with sides on fin: one who get symptoms by high converting estrogens but AR are not completely shut down by it, or just a few. Other do get more or complete AR shutdown from sudden changes. So i guess im (fortunately) in the first category perhaps.
What I did to reduce my estrogens were 2 things: keto diet and cruciferous vegetables and light regular excersize. This diet and lifestyle can reduce body fat and aromatise, as well as detoxify your liver of excess estrogens.
So for me perhaps my estrogens have started to descend to normal level again.
As of the FSH level. It could be the reason indeed, I have FSH on pfs blood tests in the lower quarter. This value on the other hand could have been by high e2, which signals FSH not to produce more T’s since there are already high estrogens.

So you recovered T, LH/FSH levels even with shrunk testicles? I think this is possible though. Sertoli cells mainly depend on seminiferous tubes for healthy sperm production driven by FSH. From my reading each published study for the last year these tubes are the most fragile and thelr structure damaged the most easily causes volume loss when they are danagee. I am curious to understand your sperm count motility levels whicih I suspect didnt return.
I am just hoping we didnt have cell death at this point.
Also in balls size i noticed kind of fluctuation. Before having stools they shrink to a minimum. But after it, within a few hours they get a proper hanging, even if with smaller size than before, but with healthy appearance.

My theory is that the size of the testicles depends on the level of gonadotropins, mainly on FSH, since most of the volume of the testicle consists of sertoli cells, the growth of which depends on the level of FSH. In the body, the level of gonadotropins decreases and therefore the testes begin to decrease in size to the current level of gonadotropins. When I first tried hCG, the testicles were then still medium in size and then they responded to hCG not badly and increased in size. But after some time, the effect of hCG disappeared and it ceased to affect the size of the testicles, only the effect on the level of testosterone remained. And this is not surprising, because hCG mainly consists of LH, because hCG mainly affects the level of testosterone produced, but not the size of the testicles. I think everyone with small testicles also have low FSH levels. And if my theory is correct, then we should go with such testicles further, because it is difficult to raise the level of FSH for a long time, if only to take clomid. I took Clomid more than once, but I did not notice any changes in the size of the testicles. But for this I would like to hear data on the levels of gonadotropins from other participants in this club))) We are waiting for your results

My testicle behavior is exactly the same)) During the day, their size changes. Perhaps this is due to a pulsating FSH, that is, its level changes many times a day. I also noticed that after orgasm they increase in size and become as dense as possible. Most likely this is due to increased production of gonadotropins during arousal and blood flow
Is it accoumpqined by testicular pain for you?
I have pain, but for another reason, I have a testicular cyst. When she was a little pain there was no pain at all, but now she is, but not strong and not constant.
Your gonadotropin level is also normal, but low levels. My FSH is a little worse than yours. And so I think this is the main reason for the decrease in testicular size.

I brought this upto my doc again during penile shock therapy today, he reaffirmed the volumes being good and long time lower FSH/LH wouldn’t cause shrinkage. I am almost certain my left one is half size or less what it used to be…We are in unknown waters…

Does your doctor have experience monitoring patients with long low levels of FSH and LH? I would be glad if he was right, then we can assume that the size of the testicles decreases due to the insensitivity of androgen receptors. I cannot think of other reasons for the reduction of the testicles.
How would I know mate? I have talked to his professor as well, his feedback is the same.
My point is even if it is lh/fsh related it is not recognised by urology medicine at the moment.
Also note there are many long time borderline low T, LH/FSH PAS/SSRI sufferers who did not report shrinkage.
Root cause seems more complex than the lack of lh/fsh.
As far as I see from stories in this forum, it is mainly PFS and SP sufferers that report shrinkage.
Maybe we can collect some data amongst shrinkage sufferers and see which parameters and meaningfully correlated.
Another point is I am trying to push myself to thinking the shrinkage might not be all too bad (as long as you dont look down and apart from the constant depressive state of feeling of abnormal, feeling of loss, suicidal thoughts etc.)
a- if we can get T levels higher- I think this might be possible since doc says my testicle volumes are good. (I completely disagree of course, wish I had some sort of record of testese volumes before SP/accutane use.)
