My E2 was 107 - and the report said it should be below 150…

My earlier E2 once was recorded at 37 on a scale with upper limit of 41 . Again the Doctor felt nothing to worry. What do you reckon? Is it normal? In 2007, 6 months after my crash I was identified with hypogonadal T levels - below the lower end of the scale for adult men.

Well your estrogen is about the same as mine. And interestingling on the scale to 40 I was also near the top. Do you feel high estrogen symptoms or high DHT symptoms? How is your hairloss?

I have all the classical symptoms of low ejaculate, genital shrinkage, severe ED and penile curvature. I dont know what are high E2 symptoms. I used to say my libido is low but I doubt i can say that really because i masturbate once a day atleast. If blood flow in my genitals improve, then I masuturbate atleast twice a day. I no morning erections though.

I slowly realise that what i call as poor blood flow to the genitals is what everyone is calling genital numbness- even if pinch my foreskin hard it does not pain. this numbess has been there since the day i crashed 5 years ago. it does improve (sensitivity) when I was on TRT and my serum T level went middle of the range and my bio-available whent top of the range. I am no more on TRT since 2 years though.

Only positive aspect compared to some other here is that my balls have always fairly been in good shape. On TRT they did become smaller though. My hairloss continued for 5 years since I crashed in 2007 but no suddenly my hair seems to be growing thick for last 2 months. I cant say for sure though.

Prime symptoms 6 years after quitting Finasteride

  1. Erective Dysfunction – clear case of impotence is evident as I could not successfully penetrate during two attempts at intercourse last year. I used Cialis on both occasions. Just did not work and has not worked for six years. I get soft erections that is no good. The best of the penis does not have strength
  2. Genital and pelvic numbness and shrinkage: This has been the same for 6 years. It did improve a bit when I was on TRT. But really bad. The main point is that its not the penis and the scrotum alone but the whole pelvis (below hips and perineum). I cannot feel even if I pinch myself hard.
  3. Penile curvature to the left: Occurred in 2008 and it’s been like that since then.
  4. Poor urine stream: spraying effect, hesitancy and poor control is evident for 6 year. Did feel like improved for a limited time when on TRT and when my sexual function as improved for a few weeks
  5. Fecal incontinence: I am made diary notes of how many times I am peeing and passing stools in a day. I was astounded to note that I pee atleast 7-8 times in a 24 hour period and pass motion 4-5 times. I always have the feeling that I need to defecate. I have had since 2007 but it got worse over the years. When I get up early in the morning, I cannot bear to control myself for more than a few seconds and have to defecate. The control is missing.
  6. Memory issues and recall rate: I realized my sudden lack of memory when I was talking to a friend in 2008 and I could not recall a famous hill town I had visited 1 year ago. I completed a post-graduate level course last year and I could not recall a fundamental principle completely. I felt frustarated and had to look it on the internet when I went home. This issue has been a challenge off late and I am losing confidence in myself
  7. Clouded Thought process (brain fog?) – My mind is consciously running thoughts. I am unable to calm my mind at all. No matter what. This makes me fatigue quickly, feel stressed even after 6-7 hours of sleep and basically makes me unproductive. I cannot read one page of anything without feeling distracted. Its so dramatic that I stopped watching movies in the last 6 years. I might have seen like 20 movies in 6 years. That’s all.
  8. Mind numbness: Overall happiness or sadness Is generally subdued. Not emotional as I would like to be.

I can keep going, talking about watery ejaculate, pre-mature ejaculation, no sensation of orgasm, really soft pelvic area in feel including penis and scrotum.
However, if I could ask for two things that I need relief with, it would be genital and pelvic sensation and Good erection. Sexual function can set me back on track a bit. Even after six years I am not losing hope as I have not had chance to meet Dr.Jacobs or other big doctors that some of you have had opportunity to meet.
I feel that each case is unique and needs to be understood separately. For example, I have had brain fog issues, sleep problems and anxiety issues even prior to starting to use Finasteride. I have been on SSRI’s for 18 months or so earlier. So, my case may not necessarily be same as some others. Or it could be. I don’t know yet.
I have come across many people on this forum with poor urine stream or fecal incontinence like I have. Very few have it. I am thinking this is neuropathic in nature. Can Hypogonadoropic Hypogonadism cause poor genital sensation?
I am meeting a Urologist next month who will review my bloodwork, ultrasound results to see if prostate is playing a part.
Regarding Dr.Alan Jacob’s post in Jan 2013 stating his results treating 300 PFS patients, I don’t feel people in this forum must undermine his words. This forum has a few thousand members wheres there are hundreds of thousands of men using finasterid. I am sure they could have consulted with Dr. Jacobs. He is not claiming cure. I just feel its not a disease so no one can claim cure. He did say he has found lot of success though.

I have found help with Gingko biloba, ginseng, L-Tyrosine and Fish Oil for racy thoughts. It keeps me calm a bit. Just started 10 days ago, so dont know if it will last.

Low testosterone symtoms. I too have the toilet issue. As soon as I get up I have to almost run for the bathroom. It got much worse when I quit TRT and let my testosterone drop to almost 0 - MUCH MUCH worse like all my problems. So for me it is simple loweing testosterone makes all my symtoms worse - Increasing testosterone without HCG improves some symptoms for about 1 week but then things get worse as my estrogen rises and I ef up with mid range free test and over the range free estrogen.

Did you post your experience with TRT in the androgen supplementation thread?

wrong conclusion. his DHT it natural, not exogenous.

My experience with TRT supplementation is here on this tread itself. I was on Oral pills…so it took a while (around 2 weeks or so) to start feeling good…and the improved genital sensitivity and urine flow would last for a few weeks and tapper off even while I continue on TRT.

I have to note that my physician did not monitor E2 regularly. Maybe that is an issue…

Currently my DHT (6 months ago) was at the top of the reference range. Why did you feel running to the washroom is a indicator of low testosterone. What do you mean when you say run to the wash room in the morning? to pee or to poop?

My genitals are shrunken like hell and I have not so good control to pee and to poop. The pelvic area seems hollowed out with soft mushy like feeling rather than firm muscle/flesh. I have not heard anyone directly connect this to Testosterone, so please can you clarify.

My own feeling is that due to extreme stress on that one night 6 years after quitting, i have had a neuropathic failure somewhere. I was below reference like for T then but it was brought up during TRT. But certainly on the lower side of reference range. The endocrinologist (new one) refused to treat me with hormones after bloodwork and said he cannot do it all looks normal.

I run to poop every morning when i wake up. This got 10 times worse when I crashed my t to almost nothing after quiting TRT all my PFS symptoms got worse when t is way lower. Life is hell then.

You have the typical response to TRT - Feeling good then your levels reach the relative post finasteride equalibrium so you may feel even worse. I have done tests testing this and even on a very low dose of trt 28.5mg test e per 3 days FREE e is over range while free T is just mid range.

I am not sure exactly what u mean by saying your pelvic region is hollowed out. For me when things are bad i feel like my intestines are not really holding on to their contents.

If TRT made you feel better for a week or 2 that is a very important factor to consider you must question why.

Too high e2 comapred to free t will give you digestion issues there is no doubt about that. I am not sure exactly how but if your body is not in an anabolic mode it dosnt really ned to abdorb much of what you eat. Also they say that is excreted in your crap and can be reabsorbed from your bowels so maybe that could also explain digestive issues.

Hollowed out…there is no bloodflow in the pelvic region and lack of sensitivity.

Here the funny thing. I have started taking Bcomplex, L-tyrosine, ginseng and fish oil for last two weeks. After one week, my penis is not as retracted as it was earlier and I feel a bit more sensitivity in my balls. So just better erection too…although its not good enough for a penertration.

Why is this happening. I know even this will tapper off after a few weeks.

i am desperate for help! I have urinary and fecal incontinence, shriveled penis…erection is so weak,…the whole pelvis is numb. How much is incontinence related to androgens? I feel I might be among few people with fecal incontinence.
If I wanted to meet Dr.Alan Jacobs, what other expenses might i incurr apart from the $600 consult feel. I need my sex life back…its been 6 years!!!

Have you considered trying pelvic floor therapy (physiotherapy which includes internal massage) to address these issues? It might be worth a shot in your case.

Thanks Komas. Anyone in Toronto area to recommend a physical therapist they have met for pelvic floor issues? Looking back, the best that I have felt is when my Total T was at around 5 on a scale of 2.8 - 8. Alas my doc stopped the replacement and put me on HcG then. I am ready to meet someone who is ready to try anything with me. If it means flooding my system with supraphysiological T, so be it. Its unbearable to live like this. Forget getting sex, I have no courage to evey pursue or feel attracted to a woman.

I could not bear the pain of a prostate message when the nurse tried to extract prostatic fluid. I collapsed within 45 seconds. Will physical therapist do it equally bad…

Hey Mark… np,

I can’t recommend anyone as I haven’t tried this route yet (although I will be giving this a go in the near future). Your GP should be able to refer you to a competent physiotherapist in your area that specializes in pelvic floor issues. As far as the pain I honestly don’t know bro… I’d imagine that a physio is likely better able to modulate the pain/discomfort associated with internal massage better than a nurse can (no knock on nurses).

Give it a try man. I suspect that pelvic floor therapy can be of great use to folks like us who clearly have some issues in that region related to muscle tightness etc. Good luck and keep us updated.

I got to meet my urologist with my blood work/urinalysis and Ultrasound results. I had hopes of so much in terms of what he could do for me alas! I was disappointed. I received some startling results though. He confirmed bacterial prostatitis and put me on 1 month antibiotic regimen. I told him about fecal incontinence. He said my liver and bladder look fine and I should request my family physician to get a referral to see a gastroenterologist if I needed. I requested a copy of the report which he graciously shared with me. I was startled to see that my total T was below the reference range. How ever the Free Androgen Index is high because of low SSBG. I feel this free androgen stuff is not trust worthy. I have such bad symptoms. I will start my antibiotic treatment today.
FSH – 3.9 (1 – 11 IU/L)
LS – 4.2 (1-8 IU/L)
Prolactin – 8 (4-15 ug/l)
Total T 7.8 ** Low ( 10 – 28 nmol/l)
SSBG – 8.1** Low ( 14.5 – 48.4 nmol/l)
Free Androgen Index 0.96 High ( .22 - .86)
Estradiol 136 ( 50 – 218 pmol/l)

2 days of antibiotics has not changed much difference to my genitals physically but I can feel my prostate a bit more. Especially when i laugh, i can feel the prostate differently. Can anyone take a shot at analysizing my Total T Vs Free androgen Index shown above and tell me how to interpret it?

This is great news man! At least you have some kind of diagnosis to run with. Excellent stuff… really hope that treatment pans out for you. Did your doc get prostatitic fluid from you? If not what was the basis for the bacterial diagnosis? Just curious.

I’ve read that it can take weeks (even a few months) of consistent rotating antibiotics to root out bacterial prostatitis sometimes. Be patient bro… you’re on the right track.
