You might want to see if things improve before you just go having surgery on your testicle on the basis of just one doctor’s opinion.
Anyway, try to just get the blood and flow moving in there first, before resulting to surgery… idk, just my opinion.
Also, have you considered a short (i.e. 1-2 months) cycle of TRT gel or cream or something. According to some of the new reports Mew has pulled up, testosterone increases blood flow. This may help some of us get over the hump while our bodies slowly recover.
I wish I had tried it, and still want to.
Unfort now I have prostatitis, which I blame on Fin which caused things to slow down just enough for just enough time for things to get infected. Thats my theory.

Do also keep in mind. It is normal for testicles to constantly fluctuate in size.

Clearly if your hormones are low and out of range, and since Finasteride messes with hormone levels, that is what I’d look to rectify. The rest of the issues may resolve on their own once you get your levels sorted out. Joint pain is a symptom of low T, btw.

Shouldn’t a testicle getting bigger be a good sign that they’re starting to produce more testosterone?
