My blood testing result, please help a newbie.

T 8.11 ng/ml (ref: 1.66 - 8.11)

Doctor did not mention the T is total T or Free T.
Is E2 too high? If yes, what next action should be taken?

Side effects:
Muscle wasting & weight loss (10kg)
Persistent chronic fatigue
Loss of Libido.

E2 is pretty high. Since you have muscle wasting despite high Total T levels, you probably have high SHBG too. Your body tries to “hide” the testosterone it produces by

  1. binding it to SHBG and
  2. diluiting it with E2

Your labs, together with the fact that you have fatigue, indicate that your cortisol levels are low OR your thyroid levels are low (very likely both). Talk about improving cortisol levels and thyroid levels with your dr.

ACTH 8AM 87.30pg/ml (ref: <46, the average is 24)
ACTH 4PM 76.70pg/ml (ref: <46, the average is 24)

COR 8AM 24.65 ug/dl (ref: 7.2 - 18.2)
COR 4PM 13.27 ug/dl (ref: 2.75 - 5.65)

I will get my 24hour urine testing tomorrow.

So scared, why got so high ACTH and COR (cortisol )? What’s the next actions?

That is an impressive ACTH level. Why were you on Proscar, do you have severe prostate issues?

The elevated progesterone is an indication that 5 alpha reductase type 2 has been downregulated (in addition to T to DHT conversion, 5 alpha reductase type 2 converts progesterone into allopregnanalone).

Your FSH is less than 3 (normal is around 5 +/- 1) this is common to those with post finasteride syndrome regardless of testosterone status.

There is a paper on diagnosing 5 alpha reductase type 2 deficiency by a group of Chinese doctors “Diagnosing 5 Alpha Reducatse Deficiency, A Local Experience”:

Would recommend you see them and have them evaluate your urinary metabolite ratios following the method outlined in the paper. Would be quite curious what they think of the findings.

you’re e2 does not look too high.

your adrenals are def. going overboard. this happens in the first few months/year after the crash in many people, including me. it explains your fatigue, but wait untill they are worked overboard and crash, your fatigue will be worse. if you are lucky you might be able to stop it in its tracks. its hards to know for sure because we do not know the reason they are doing this. it is not auto-immune, it may be the hypothalimus or the pituitary, or you might just have a bad infection (this is likely) like prostatitis.

try taking some adaptogens like rhodiola and ashwaghadia.

in the mean time get your t3 and reverse t3 tested.

It seems my Progesterone and E2 are rising and T is downing compare the previous blood test, what the hell mean???

Also, my new FT3,FT4,rT3 result below:

FT3 2.77 pmol/L (ref: 2.30 - 6.30)
FT4 16.76 pmol/L (ref: 10.30 - 24.50)
rT3 0.763 nmol/L (ref: 0.24 - 1.46)

FT3/rT3 = 2.77/0.763 = 3.6, the 3.6 is below 20, does it indicate a rT3 issue?

Besides, I retest the ATCH and COR with blood, they all are very high, but I have normal cortisol with 24 hour urine test which is in range.

Current I take the zinc picolinate 50mg daily, what additional actions can i take???

Your main issue, I think, is a too low FT3. You have high cortisol, which keeps your RT3 optimal, but consumes all your available thyroid hormones. (Remember that cortisol and T3 work together to increase energy and metabolism, and they must be balanced). The same thing happened to me a few months ago and I treated it with T3 (cytomel). That will definitely lower your cortisol. If you are lucky, it will also decrease E2. You may want to talk to your dr about this.

It means that at some point during the day your cortisol tanks. Anyway, the urine test is in range but on the high end. Therefore, you have high cortisol.
