I have been having terrible fatigue and weakness of recent

Be careful. Supplementing with folic acid/folate is a bit of playing with fire - especially for us. Assuming you were taking methylfolate, you were taking the risk of a so-called methyl traps.

In my opinion, the best (and above all safe) solution would be to supplement the deficiency through diet. I mean eating green leafy vegetables (spinach, lettuce, parsley), broccoli or beetroot.
I ignore synthetic folic acid (not in the methylated form), because taking it in supplements is pointless (it must transform into active forms, and its excess is toxic).
my doctor has prescribed me 5mg folic acid a day and asked that i return for bloods in 3 months
