I need your help in creating a questionnaire

Hi everyone.

Something which is missing in this forum is to organize the info and results in one place, in order to be able to correlate data.

I hereby propose first to do a questionnaire, where we would store the info on a DB. Later on we could try to work out some statistics.

I’m a software developer and I could possibly make a public accessible website for the questionnaire.

I have made this diagram and I would like to have your opinion on it and what should be changed/added.

While I applaud your initiative, what is the purpose of this?

There have already been various studies published using statistical methods and questionnaires.

Questionnaires are not going to solve our problem, research will. Pls help raise awareness of PFS, and fundraising for the PFS Foundation to be able to help facilitate studies into the problem.

We make discoveries by finding patterns. If the survey is comprehensive and enough people take it, then perhaps there will be some interesting pattern in the data and any such patterns can help direct studies.

I find that we can raise awareness and do this at the same time.
I am personally finishing a letter to the media newspapers on my country to try and raise awareness.
I think patterns are always important to search for and if in the end its lost time because we didnt find anything, at least we have more data. I’m not asking for help in developing it, I would do it from my spare time, it will be a big effort from my side but i am willing to do it, but if you have any questions to add or change in my questions diagram flow, please feel free to contribute. that’s what this post is for.

do you have brother, do you have sıster, have you or your famıly member mgrain, asthma, parkınson, hand tremor, allergies, tachıkardya, epylepsi, seborhaic dermatitis, did you suffer testıcular paın or hormonal problem ın your chıldood?
Do you use rıght hand or left hand for wrıtıngs?
Do you ever use beta blocker, alpha blocker

I’d add a question to establish Norwood status. I’ve wondered if those without crown balding (i.e. hairline recession) are more susceptible to side effects.

I’d add an image or link to a Norwood chart, in case people aren’t sure. Try to keep the questionnaire as simple as possible, while any complex info can be volunteered.

I can appreciate this flow chart concept. I’m also an IT guy and familiar with development and databases.
However, I’ve already seen so many studies, reports and scientific data which has proven the dangers of finasteride.

I think if you use this same concept, but gear it towards collecting data on all the various things guys have tried in order to recover, it may help to identify what works and what doesn’t. The items could be categorized and have subsets of data points.

ie;
Exercise (what type, duration, frequency, results experienced)
Diet (items to avoid, items to include, daily intake etc, duration, perceived results experienced)
Supplements (Name, Mfr, dose, frequency, how long taken, results experienced)
Vitamins (same as above)
Medications/Creams/TRT (Name, dose, frequency, etc)

There is already a wealth of information on this site, but it’s scattered and includes modifications posted afterwards which tend to change over time.

If you took this route, each person could access the site or database to make log entries to build a data set which could be analyzed for common themes of success, or not. So I think you have a great idea, only I think if it were geared to become a helpful and useful source for guys to utilize for recovering.

I would have a lot of info and experience to input after trying so many things over the past 4 years. I’m on year 5 and doing much better. And I can point to specific things which contributed to improvements.

Hi towm8er, it’s exactly that what I want to do, in addition to having more details on the crash and things before and after the crash.
If you see the right-most part of the flow chart you will see the Treatments related part of the survey and you will see exactly what you posted.
I want to give everyone a login and everytime we’re adding new questions, or modules of questions, everyone should be notified and called to answer the questions.
then the data could be exported to an excel spreadsheet by anyone and everyone could try to find patterns.
