Hi. Thank you for having posted that whalen72, your mention of the Mayo Clinic was very helpful. Hospitals in Common may have been able to work out for me, but the price would have been a serious burden the way my finances are at the moment. Iām hoping that I have found a cheaper route.
I called the Mayo Clinic Medical Laboratory and had a very positive experience. Despite that the line is most likely intended only for doctors, the woman with whom I spoke was very friendly and informative, willing to clarify everything for me. What I learned from this will be extremely valuable, because my doctor usually politely stresses not wanting to get into anything too complicated or drawn out, and she clarified that it shouldnāt end up being so.
Now the steps from my end seem to be:
1- Bring my doctor a printout of these steps:

The first thing this process will do is help him start an account. They were able to clarify that making one is not complicated and will not cost anything, which is important. It will allow him to order specimen transport supplies and requisition the test.
2- Bring him a printout of Mayo Clinic labās specific listing for the test:

After his account is set up, the unit code (80227) alone should make calling and asking for the relevant supplies simple, she said they will likely know what to send him. I also gather that they would also be able to figure out what requisition form he needs pretty easily (not so easy to find on the website), and would be able to fax it to him.
3- After the supplies have arrived and he has filled the form and we have all the info on where to send to, I should be able to bring the supplies and the collection and transport guidelines and get my blood drawn at the lab downstairs. If all goes well, they will know what to do with the supplies and get the kit all set up and ready for FedEx, who should be able to pick up from the lab itself apparently.
4- He will be billed for the cost of the test, so I pay him specifically. We have had similar arrangements before in the past when certain drug companies have sent him invoices for currently unapproved psychiatric drugs we have requested through the federal Special Access Programme. I donāt think this arrangement should make him too squeamish.
So to try to figure out how much I have to pay, it seems to come to this:
-cost of test itself through mayo clinic ref lab: $254.50 US
-cost of supplies: initially waived, included in the above cost
-cost to have blood drawn at lab: ~$25 CDN i think
-cost for package setup/freezing before transport if needed: unknown
-cost for transport via FedEx: unknown
Is there anything I seem to be leaving out? Could I be charged any tariff or duty on this somehow?
At least so far, Mayo Clinic Medical Laboratories seems to be a pretty impressive lab. They may be worth adding to the list on other stickied threads that mention Labcorp, Quest, and ARUP, especially given that the price ($254.50 US) seems to be more reasonable than other prices I have come across (over $300 for labcorp, iirc.) I will keep you guys posted on how everything goes.
As a side note, Iām pretty happy that I was able to concentrate and type this all out and so organisedly. I think the Concerta (sustained release methylphenidate) is helping and might be a really good sign. (I have serious tolerability issues with Adderall and dexamphetamine.)
