So I ended up stopping the clomid + anastrozole because I was experiencing pretty bad mood swings, subtle brain fog, and blurry vision. Moreover, both clomid and anastrozole caused pretty severe scalp inflammation, which was actually unbearable. I couldn’t even touch my hair without my scalp burning like crazy. I also wasn’t really comfortable with lowering my estrogen because I never felt like it was suboptimal for me, as I’ve maintained good libido and erectile quality (although I haven’t gone a day without cialis). Additionally, my new labs are:
Testosterone (total): 1,205 ng/dL
DHT: 83 ng/dL
LH: 10.1 mIU/mL
FSH: 5.7 mIU/mL
E2: 31.1 pg/mL
SHBG: 41.3 nmol/L
To be honest I was not expecting my T to be this high after only 2 weeks of treatment (I even skipped some doses). Having that much T really didn’t feel that great for me, which could be because of the high SHBG. Anyways, my issue at this point is not a lack of androgens, but rather structural.
I went and saw a proper urologist who specializes in pfs. After performing the ultrasound they found diffuse fibrosis in my penis. Not full blown plaque, but fibrosis scattered in the corpus cavernosum, particularly the left side. I was more relieved than shocked, because at this point I’m just mentally exhausted from this whole situation. I went so far into handling my hair loss that for the first time in my life, I damaged my body significantly. So far it’s been painless, but I imagine things could’ve gotten a lot worse had I ignored the signs of structural damage (hourglassing, shrinkage, etc.).
In regards to treatment, I will be doing shockwave therapy. I know there has been mixed success with this modality of treatment, since it’s quite experimental and cost-prohibitive, but I really want to find a cure for myself so I can find some peace and move on with my life. My erections are quite good with cialis, but I’ve lost a bit of girth, and I attribute that to the mild fibrosis that is probably restricting blood flow. As for the “hourglassing,” the doctor told me that because it’s not present during full erection, then it’s not caused by the fibrosis, but I’m a little apprehensive about this still. The shape is present just before a full erection and when I’m flaccid or semi-flaccid. It’s also exacerbated by bowel movement and when I contract my penis. It’s not the end of the world, but it’s still quite annoying and worrisome. I’m hoping that it may go away with the shockwave treatment, but since it’s not a functional issue but rather cosmetic, then it shouldn’t be a top priority right now. Does anybody have knowledge on the hourglass shape? I know there is a difference in the underlying issue depending on whether it’s present in the fully erect versus flaccid state. Same goes for the loose scrotum. He said that down the line I could see a plastic surgeon for it if it really bothered me. It does, but we’ll see if this one recovers naturally. I will post an update after the shockwave treatments. Let me know if anybody has any questions, I’d be happy to answer them.
@tgfbeta I have the same physical symptoms (girth loss, curvature, hourglassing) but NO fibrosis. Did multiple imaging tests and they found nothing. So maybe something else is causing the same issues with you instead?
