Hi @iHealth, there should be instructions on how to do this in our New Patient kit on the propeciahelp.com homepage. Look for the “New Patient? Start Here” button on the right.
I looked at the FDA site and I’m not sure about it. I have been to the emergency so then do I check that I have been hospitalized. I’m not sure do they want like document etc.
From memory it was pretty straight forward to complete. You don’t need any hospital evidence or anything, you just submit an adverse reaction report. The instructions should be in the new patient kit.
