Can anyone help me decipher these lab results?

Background:

I’m from the PSSD forum (fluoxetine-induced PSSD going on 3 years now), and my health has been pretty shitty for a while. I’m 24, exercise regularly, and eat mostly paleo. I believe I have issues with both my HPA axis and HPTA axis.

The most improvement I’ve seen is from Naltrexone (high dose - 25+ mg) in conjunction with boron (~12mg). The degree of improvement faded, but I feel they still help. Now I may know why…

I just got my lab results in.

I would greatly appreciate it if anyone could help me interpret the following results, as well as give me some guidance with regard to how to improve my numbers (and consequently my sexual function).

Note: Test taken at 12 noon while somewhat stressed/anxious (probably high cortisol/adrenaline - hate doctors)

Testosterone, Serum 532 ng/dL 264 - 916
Free Testosterone(Direct) 8.8 Low pg/mL 9.3 - 26.5
Dihydrotestosterone 49 ng/dL [ Reference Range: Adult Male: 30 - 85]
Prolactin 6.1 ng/mL 4.0 - 15.2 (on low end of spectrum)
Estradiol 7.8 pg/mL 7.6 - 42.6 (on low end of spectrum)
Estrone, Serum 78 High pg/mL 15 - 65

Adult male reference interval is based on a population of
healthy nonobese males (BMI <30) between 19 and 39 years old.

Took a testosterone test earlier in the week, but I was stressed and it was taken in the late afternoon (4pm), so I don’t think the results were reflective of my normal levels. As such, I had it redone (which are the results above). These are the results from the first test:

Testosterone 3.7 ng/mL|2.2 - 7.8 ng/mL
Sex Hormone Binding Globulin 51 nmol/L 13 - 90 nmol/L
Testosterone Free 52 pg/mL 30 - 140 pg/mL

Is my SBHG just way too high? Is that why my free T is so low? I’m guessing the improvements from Naltrexone/Boron combo were due to increased free T. I felt like my old sexual self for a period there, but then I seemingly became tolerant to the drugs/supplements.

My prolactin is low, which is probably why dopamine agonists didn’t help. My estrogen is low, too, which is probably because free T is low and there is not much aromatizing happening.

Is TRT my only option here?
