09eric09's Bloods (Need Feedback)

My biggest problem is dealing with exhaustion. My bloods show something is going on with adrenals, what would be my next step? A 4x Saliva cortisol? Before bloods I was sure adrenals would come back low, but they all ended up being high. Bloods indicate Cushing’s and my symptoms match (i.e. extreme fatigue, panic attacks, low bone density).

Total Testosterone - 4.2 (2.5 - 11.5 ng/ml)
Free Testosterone - 11.27 (9.00 - 47.00 pg/ml)

T3 Total - 104 (55 - 172 ng/dl)
T4 Free - 1.2 (0.1 - 1.6 ng/dl)
TSH - 1.490 (0.358 - 3.740 uiU/mL)

Cortisol - 24.8 (4.30 - 22.40 ug/dl) High
ACTH - 79 (0 - 46 pg/mL) High
DHEA - 8.960 (1.330 - 7.780 ng/mL) High

Urine Cortisol, Free 24HR - 27.4 (<=60.0 ug/d)

Prolactin - 17.0 (2.1 - 17.7 ng/mL) High

After doing some reading, I finally decided to get my adrenals tested, I just couldn’t take the exhaustion anymore. I was sure my cortisol was going to be low, but after a 24 hr urine, my cortisol is mid range. But, my ACTH was way over range, along with DHEA. What does this indicate? I’m sure there is something going on with my adrenals, but it seems it may be cushing’s, rather than adrenal insufficiency. Should I get a 4x saliva test?

Hi, I just came across your blood tests…

I need to know if you havemore blood tests, we may have a similar profile as I see your prolactin level is around the same as mine, and there is a change that then Progesterone and Estradiol will have similar results too. I think the cause for our problems is in the negative feedback loop of 5ar with DHT, Progesterone and IGF-1.

If you want to see things off range here’s what you need to test:

  • Cortisol in saliva 4x a day
  • RT3/T3 ratio
  • IGF-1
  • Progesterone
  • 17alpha Hidroxiprogesterone

Check for congenital adrenal hyperplasia. The rare forms too. Preferably with an ACTH stimulation test, and calculation of precursor/product ratios.

12 year sufferer, 34 M, crashed from Accutane at age 21.

First few years post-crash were awful, no libido whatsoever, then small increases here and there in my mid to late 20’s but nothing close to pre-crash.

This year I have experienced the highest sex drive I’ve had since crashing many years ago, not back to normal, but the closest I’ve ever felt.

Oddly enough, this year I started losing hair on my crown and my free testosterone is the highest it’s ever been since drawing bloods the last 12 years.
